As director of Systems Design at the Center for Globalization and Sustainable Development, we work on integrating community generated health information (i.e. personal health devices, community health workers, community organization reported) with health information systems and payment mechanisms. Our goal is to improve primary health care system delivery by making communities integral and active parts of the health care system. This includes the real-time analysis of time-series and location-based information about individual health, with a focus on depression, diabetes and hypertension. We utilize information about individual’s social ties and personal goals to achieve long term chronic disease management. Our partner for this work in New York City is "City Health Works!" ( They are a non-profit social business that was designed to facilitate scalability of the rapidly trained community health workers who deliver the Diabetes Prevention Program and support population management functions. Our approach has been recognized by the Robert Wood Johnson Foundation for its innovative use of low resource delivery methods and technologies, which we have pioneered in 10 countries across Sub-Saharan Africa.Faculty page:
